Your Hospice Team
The Hospice Team consists of Registered Nurses, LVNs, Social Workers, Chaplains, Nurse Aides, volunteers and Physicians. Our team works together with a holistic approach to care for our patients and their families.
Hospice also provides on-call nursing services to families 24 hours a day, 7 days a week.
Where We Provide Care
Southern Grace Hospice provides care to our patients where they live. Our services are rendered at your home, a skilled nursing facility, assisted living facility or group homes. Our staff is experienced at helping families find quality placement if that setting changes.

Hospice services include: medical equipment, medications, medical and personal supplies, wound care, medication teaching and management, spiritual care, advanced directives, counseling, and all types of palliative therapy.
